The NHS requires that all patients with certain conditions have an annual review and by completing your surveys online it saves both you and your GP practice time without cutting corners. You still get the treatment to help you manage your condition but without having to attend the surgery and at a time convenient to you. 

Right now Medloop helps your GP practice complete their reviews for Asthma, COPD, and Hypertension – but we are working with GPs and nurses to find new and better ways to help patients with other conditions too.

You might be wondering who Medloop are and why your GP surgery has joined up with them. We’ve put together this Q&A to answer any questions you might have about us and the service we are helping your doctor provide.

You still get the treatment to help you manage your condition but without having to attend the surgery and at a time convenient to you.

Right now Medloop helps your GP practice complete their reviews for Asthma and COPD – but we are working with GPs and nurses to find new and better ways to help patients with other conditions too. This won’t happen just yet – so if you usually see your practice about something other than asthma or COPD they will get in touch with you separately. In the future, we hope we can complete reviews for different illnesses at the same time – including ones which need measurements and tests. We’re working it all out to make it as easy as possible.

When you fill in the questionnaire your scores are calculated, according to official guidelines for your condition. Depending on your result, you may be offered a chance to book an appointment straight away. This will take place with a member of the Medloop clinical team who can contact you by phone or conduct a video consultation. If your condition is well controlled you will not need to have an appointment, although one of our team may invite you for a check up if they have any questions.  Everyone completing an asthma survey will be sent a care plan which you can access using the link in the text message.

Medloop integrates directly with the software your GP practice uses so your medical score, any notes from an appointment or any changes to your prescriptions will also be included in your GP notes along with your care plan.

We’ve thought of that!  We know not everyone can fill in surveys on forms or complete appointments by video which is why you might receive a call from one of the team. If you need help filling in your answers our team can do that with you – and if needed book you an appointment at a time which suits you. If you don’t have a way of having a video appointment – or would rather speak to someone on the phone, that’s not a problem. Our nurse will always call first and if it’s something you need to deal with in person, we can arrange that with your GP practice.

You can fill in the survey for someone you are caring for as long as you have the link, their date of birth and their approval. If the patient needs an appointment but you think they need will not be able to manage a phone or video appointment, we can make arrangements.

On the text message you receive confirming your appointment, there is a link to change or cancel. Follow this link and you will be able to cancel your appointment. If you do need to cancel, please allow as much time as possible to allow another patient to use that time slot. 
If you need to change your appointment you can contact Medloop on info@medloop.co or call us on 0330 818 0062 and we can make those changes for you.

If the practice needs to cancel your appointment they will be able to rearrange directly or send you a link which allows you to book another appointment when it is convenient for you.

Simply click on the link included in the text message and this will open the review in your browser. Should you prefer to complete your review on a desktop computer, just type the link into your browser. 

Once you are on the page, you will need to give your consent and enter your date of birth to confirm your identity. 

The questions we ask are based on NICE guidelines issued by the government and are the standard questions used across the NHS to monitor your specific health condition. 

Medloop prioritise which patients need to be seen first depending on the unique responses given during the review. Appointments for consultations are offered based on clinical need and if we calculate that you do need to speak to one of our clinical team you will be shown the available appointment times at the end of your online review. Because our staff are dedicated to providing you with your reviews we can often see you quickly for any follow up consultations. 

Follow up consultations are not always necessary and depend on your symptoms and how well you are managing your condition. However the team who hold consultations may invite you to an appointment if they have any questions about your answers or if you have raised concerns in the review.

Once you have completed the survey, you will be able to book a remote consultation with a nurse. This is not always necessary and depends on your condition. If you did not get the suggestion to book a consultation but you would still like to talk to a nurse, contact us at info@medloop.co and we will get back to you!

We guarantee a very high level of protection of sensitive data. We have been awarded ISO 27001 certification. Medloop may not pass on any data to third parties and is fully compatible with the GDPR guidelines.

A SMS message is sent to the patient following their review with a link to a PDF of their completed care plan; this can be accessed at any time. A copy of the care plan link can also be found in the patients’ EMIS record.

Please open the latest text message you have received from your GP surgery, which confirms your remote consultation. Please double-check the time of your appointment.

When it’s time for your consultation, please click on the link in the text message to join it. You can enter it directly from your phone.

You are now in the virtual waiting room. Once the nurse has entered, the screen will change automatically, and your consultation will start. 

Should a few minutes pass and nothing is happening, please try a hard refresh of the waiting room page. You can do so by closing the window and clicking on the link one more time. 

In any case, should our nurse not be able to reach you via the video consultation, she will phone you instead. So don’t worry!

If you do not have a blood pressure monitor or recent reading, we cannot carry out a remote consultation in this instance. Don’t worry, your GP surgery will contact you directly at a later date to schedule you a face to face consultation. Please note that there is only one question in the survey if you do not have a blood pressure monitor.
